- Annual foundational physical which includes advanced lab work, bone densiometry, cardiovascular stress test, carotid, thyroid, liver, and gallbladder ultrasound screening, body composition evaluation, flexibility and strength assessment
- Annual consultation with Registered Dietitian
- Annual consultation with Exercise Physiologist
- Physical follow up appointment with comprehensive care plan
- Quarterly labs reviewed with physician to assess progress with care plan
- Unlimited office visits, telehealth visits, or telephone consults as needed
- Unlimited access to Exercise Physiologist, Registered Dietitian, and team
